# Why use PKB Questionnaires

- Enable patients to receive and retain a copy of their responses for reference.
- Reduce reliance on face-to-face, paper-based questionnaires by providing a secure, digital alternative.
- Improve efficiency and quality of information exchange between patients and clinical teams.

# How to Request a Questionnaire Template

- Identify which questionnaire(s) are required for your patient cohort.
