Patients Know Best (PKB) is the world's most comprehensive and patient-centred care planning tool. It captures structured and meaningful conversations about a person's care. 

This toolkit shares a library of prebuild care plans that can be used or amended to suit your team and patient's needs.  It will show how PKB care plans feature can be used to set realistic goals and clear action steps, empowering patients to monitor and self-manage their health.

PKB connects with hospital Electronic Medical Records (EMRs) and our APIs, allow for real-time information to flow in and out of the patient’s health record. Patients can read, edit, and share their care plans electronically with anyone they choose.
