# Key Objectives

To provide patients with digital care plans with information on high blood pressure and hypertension. All information is taken from NICE guidelines.

# Outcome

Equip patients with the information they need to self-manage their high blood pressure. Provide generic disease care plans for teams to have in their PKB site for patients to engage with while waiting for treatment or after diagnosis.
