There are several oncology care plan templates available in PKB, primarily focused on consent, symptom tracking, and remote monitoring. The oncology services typically combine these care plans with a range of key features:

- Advanced Questionnaires: For waitlist validation and Patient Reported Outcome Measures (PROMS).
- Library Resources: Providing education materials, patient leaflets, and support group information.
- Secure Messaging: Allowing patients to initiate contact with their speciality cancer teams and reducing administrative phone calls.

# Symptom Assessment Care Plan

The Symptom Assessment Care Plan enables clinical teams to remotely monitor patients' symptoms throughout their cancer treatment. Patients complete structured symptom at regular intervals, allowing clinicians to identify concerns early and intervene when needed. This care plan can be tailored to specific cancer pathways and combined with library resources and secure messaging to support patients between appointments.

# Holistic Needs Assessment Advanced Questionnaire 

The Holistic Needs Assessment (HNA) PKB Advanced Questionnaire provides a structured way for patients to identify and communicate their physical, emotional, practical, and social concerns at key points in their cancer journey. Completed digitally through PKB, the questionnaire helps clinical teams prioritise discussions during consultations and tailor support to each patient's individual needs.
